1. Provider responsibility. The AI chart prep tool assists with documentation. It does not provide medical advice or clinical judgment. You, the provider, are solely responsible for reviewing, editing, verifying, and approving every note before it is used or entered into any medical record.
2. AI can make errors. AI-generated output may contain mistakes, omissions, or inaccuracies. It must always be checked for accuracy and completeness by a qualified provider. Integrity Virtual Partner is not responsible for any error, omission, or clinical decision arising from unreviewed or unverified output.
3. Eligibility. This service is available only to US-based licensed providers (such as MD, DO, NP, PA, DDS, DMD, or DPM) with an active National Provider Identifier (NPI). You confirm that you meet these requirements and that the information you provide is accurate.
4. No patient information in the intake form. You agree not to submit any protected health information (PHI) or patient-identifying information through the intake form.
5. Setup fee and refund. The setup fee is a one-time charge of $499. It is refundable within 30 days of payment upon request. After 30 days, the fee is non-refundable.
6. Turnaround. We aim to deliver your completed setup within approximately two weeks of receiving your completed form and payment. Timelines are estimates and may vary.
7. No guarantee of outcomes. We do not guarantee any specific clinical, financial, billing, coding, or reimbursement result from use of the tool.
8. Compliance. You are responsible for ensuring your use of the tool complies with HIPAA and all applicable laws, regulations, and your own organization's policies, including obtaining any required consents and maintaining appropriate safeguards for patient information within your own systems.
9. Not legal or medical advice. Nothing provided constitutes legal, medical, billing, or compliance advice.
